Support Planning
Lincolnshire County County is committed to maximising choice and control for all people who require long term support from Adult Social Care.
The support plan sets out, in a person centred way, the outcomes that the individual wants to achieve and explains how they are going to use their Personal Budget to achieve those outcomes. A support plan can be created in any medium or format, but we do ask that it contains certain pieces of information.
The Support Planning Toolkit is designed to help people develop their own support plan, the toolkit is attached to the downloads on this page.
