Help at Home

Wherever possible, we prefer to help people to live at home independently with a support package. Often known as community care, services might include:

  • Home Support - Help with personal care such as washing and dressing
  • Community Meals - Usually supplied frozen to reheat yourself
  • Day Services - Available in care homes or community centres
  • Respite Care - Residential care or care provided in your own home to give you or your carer a break
  • Housing Adaptations - We can provide information about equipment and adaptations to make it easier for you to continue to live in your own home
  • Direct Payments - This scheme allows you to buy in the care you need for daily independent living
  • Supported Housing or Extra Supported Housing is provided by your district or borough council.

To decide what sort of help you need, the local authority will carry out a community care assessment and arrange a care plan. This sets out what services will be provided to help you manage at home.

Many people can be involved in arranging a care plan, but it is important that you and anyone who is caring for you take part in the discussions. Your views and wishes should always be taken into account.

It is likely that the local authority will make a charge for services it provides, based on your financial circumstances. You should be given clear information about what you can expect to pay.

Care plans may change if your needs change, leading to another assessment, which is often called a review.

Home Support

Home Support is practical help which we can provide in your own home, to enable you to remain independent for as long as possible and improve your quality of life. This is usually help with day to day tasks, which could include getting up, dressing, washing, going to bed or help with eating

We can also help you contact other agencies who may be able to help you with other special needs which have been identified in your care plan. This may include laundry, shopping, household chores, preparing meals or collecting a pension

Who can receive support?

If you are an older person who is in need of assistance with everyday tasks, or have a physical or sensory disability or a mental illness, then Social Services can arrange for help at home. The amount of support you receive each week depends upon what is decided following an assessment of your needs. We will review the amount of support received on a regular basis and adjust the amount of care - if necessary - as and when your needs change.

Who provides support at home?

When it has been assessed that you require support at home, the council provides a short term reablement service.  The Lincolnshire Assessment and Reablement Service (LARS) is a service to enable you to remain as independent as possible and to try and ensure that you remain at home with the right level of support. After this short period of assessment and reablement, your needs will be reviewed and if you need support at home this will be agreed with you through a support plan, which will also involve how that support can be provided. If you need the help of a home support service, then this can be provided by a number of Independent Home Support Agencies, who are frequently checked for the quality of their services and have been approved by the County Council.     Community meals are available and can be provided on a short term or long-term basis according to need, and after an assessment has been undertaken.
