Intermediate care aims to help you remain independent at home wherever possible. It works by providing you with a programme of care to rehabilitate you and help you to carry out the activities you find difficult.
Intermediate care aims to:
- Prevent unnecessary admission to hospital or long term care
- Enable someone to come home from hospital earlier than would normally be expected
- Help prevent further deterioration in a person's health
- Promote independence
Intermediate Care is available to people who meet set criteria. It can last from 24 hours up to six weeks, depending on individual needs, and takes place either in someone's own home or an intermediate care bed in a hospital or care home.
