The Mid Year Application Form

If you contact the Education Team direct for a mid year application pack it will contain:

• A four paged mid year application form
• A letter explaining what to do
• If we know which schools you wish to apply for your pack may include supplementary forms.  Some schools take into account medical or faith reasons and the supplementary forms will allow you to provide extra information on these reasons.

Collecting an application pack from a school

If you collect an application pack direct from a school they will download the documents from our website.  If you want to apply under faith or medical reasons you will need to mention this to the school.  If it is something they consider as part of their admissions process they can provide copies of the supplementary forms.  Alternatively you can download these from our website.

Where to return your form

Once you've completed your application form please return it to the address stated on this page, please state on the envelope that it is a 'Mid Year Application Form'.
