Register a birth after 12 months

If a birth occurs in England or Wales and has not been registered within 12 months, a late registration is possible. A late registration can only be authorised by the Registrar General if:

  • evidence of the child’s exact date and place of birth is shown
  • a person is available who can attend any registration office in England and Wales to give the information for the registration

How to apply

To apply for the late registration of an unregistered birth, complete one of the Application for Late Registration forms from the downloads section below. Use form C45 if the birth was in England or form C45W if the birth was in Wales.

The application form will give more details of:

  • the information you should provide on the form
  • the documents you may need to send with the completed form

No fee is charged for authorising a late registration. However, further certificates are available for £10.
