Children and young people's nurses

Children and young people’s nurses provide support for those aged 6 to 19, with more complex health needs, where other services are not designed to meet their needs.They also work with people up to the age of 25 who have special educational needs and disabilities (SEND).

This may include support for:

  • healthy weight management
  • continence
  • enuresis
  • managing long-term health conditions

They work closely with health visitors to ensure a seamless pathway to continued support for identified children, after their reception year, and their families. They also work closely with schools, GPs and other relevant services to help children and young people with continuing complex needs.

Access to support from a children and young people’s nurse is via referral only. For children currently in their reception year referral will be via their named health visitor, where the health visitor has identified that there is a need for continued support after completion of their reception year.
