Home from hospital

Home Safe is a transport and resettling service for people returning home from a hospital stay. It is designed for people who do not have family or friends who are able to help.

If you need help to make sure everything is ready and safe for you to return home after hospital, our Home Safe service can be arranged by the hospital ward staff. One of our drivers will take you home, where you will be met by a Wellbeing Response team member, who will help you settle back in.

Once you have been transported home, you will be met by one of our Wellbeing Response team members who will work with you to undertake tasks that may include:

  • ensuring you are able to access your home
  • turning the heating on, if required
  • ensuring you understand when to take your new medications
  • checking that other social care and medical visitors have the correct information, to assist with seamless care
  • communicating with neighbours and family, if appropriate
  • removal of any obvious hazards
  • making sure you have enough fresh groceries
  • making sure your day-to-day belongings are in the right place, i.e. making sure your spectacles, commode, walking stick or frame and Telecare pendant are easy for you to reach
