Willows Lane, Frithville

I am making this request for recorded information relating to the condition, inspection, reporting, assessment, maintenance and repair of Willows Lane, Frithville, specifically the pothole/defect referred to in Lincolnshire County Council highway claim PLPD25/00832, Council Enquiry 2871668 and Works Order 5857648.

The Council's decision letter states that the defect was reported to the Council on 22 January 2026, that a works order was subsequently raised, and that the repair was completed on 9 February 2026. Please provide the following recorded information. 1. Enquiry 2871668 Please provide the complete record for Enquiry 2871668, including:

* the original report submitted on 22 January 2026;

* the date and time the report was received * the date and time it was first viewed/processed by the Council

* all photographs attached to the report * the location information recorded against the report

* the defect description and categorisation

* the risk/severity classification assigned to the defect

* any inspection or assessment undertaken following receipt of the report;

* any notes, comments or actions recorded by Council officers

* any correspondence or internal communications relating to the enquiry

* the date and time the enquiry was passed to any highways inspector, contractor or maintenance team. If any personal information relating to the person who submitted the report is exempt, please redact that personal information rather than withholding the remainder of the record.

2. Assessment and action following the 22 January report Please provide all recorded information showing what action was taken by the Council between receipt of Enquiry 2871668 on 22 January 2026 and the completion of Works Order 5857648 on 9 February 2026.

This should include, where held:

* inspection records;

* risk assessments;

* photographs;

* officer notes;

* instructions to contractors;

* decisions regarding temporary or permanent repairs;

* decisions regarding warning signs or other temporary safety measures;

* records showing whether the defect was considered an immediate or emergency safety risk;

* records of any proposed, rejected or deferred action.

3. Works Order 5857648 Please provide the complete recorded file for Works Order 5857648, including:

* the original works order

* date and time it was created

* description of the defect

* priority/category assigned

* target repair date

* any risk assessment;

* photographs before repair

* photographs after repair

* contractor instructions

* records of attendance;

* records showing the date and time the repair commenced and was completed

* details of the repair undertaken

* any inspection or sign-off following completion

* any notes concerning the condition of the previous repair.

4. Previous repair at this location The report submitted on 22 January 2026 described the defect as: "Large deep pothole, whole of previous repair gone." Please provide the recorded history of the previous repair at this specific location, including:

* the date of the previous repair

* the relevant works order/reference number

* the nature of the repair undertaken

* any photographs

* any inspection/sign-off records

* any subsequent reports concerning failure or deterioration of that repair

* any records showing when the Council first became aware that the previous repair had failed.

5. Highway inspections The Council's claim decision states that Willows Lane is a Hierarchy 5 carriageway and that Hierarchy 5 carriageways are inspected annually.

Please provide:

* the complete highway safety inspection record for Willows Lane covering the 12 months preceding 22 January 2026

* the complete inspection record dated 14 May 2025

* all photographs associated with the 14 May 2025 inspection

* all defects recorded during that inspection on or around the location of Enquiry 2871668

* details of any repairs arising from the 14 May 2025 inspection at or near this location;

* the recorded date and details of those repairs

* any subsequent inspections of this particular location before 22 January 2026.

6. Inspection and intervention criteria Please provide the version of the Council's highway inspection, defect intervention and repair policy applicable to Hierarchy 5 roads on 22 January 2026, including the relevant sections of the HIAMP referred to in the Council's claim decision.

Please also provide the recorded intervention criteria applicable to a defect described as large/deep or dangerous, including the criteria used to determine whether:

* an immediate repair is required;

* a temporary repair is required;

* warning signage is required;

* traffic management is required;

* a defect can appropriately remain awaiting permanent repair.

7. Reports concerning the same location Please provide details of all highway defect reports, enquiries, complaints or maintenance records held by the Council concerning the same defect or immediate road location during the period: 1 January 2025 to 9 February 2026.

This should include the relevant reference number, date, defect description, action taken and repair date.

Where individual reports contain personal data, please provide the records with the personal information redacted.

8. Records relating to the Council's 28-day repair position The Council's claim decision states:

"For a Hierarchy 5 carriageway, this is within 28 days." Please provide the recorded policy or document establishing this 28-day permanent repair period and, specifically, any recorded policy or guidance explaining how that 28 day period applies where the Council has received a report describing a defect as large, deep and dangerous and difficult to avoid because of its proximity to a junction.
