5. What was the longest wait for an EHCP plan to be finished as measured from the initial assessment request to delivery of the plan during the request time period? (2014- to date)

The request or the formulation of the EHCP have been actioned but not yet completed either within the 6 week timescale for assessment and 20 week timescale for the issue of the EHCP.
