Damage to Apparatus at Church Street, Louth - Find a freedom of information request

I am writing in terms of the New Road & Street Works Act 1991 for information on which company had an opening notice to carry out street works and works for road purposes between 81 to 99 Church Street, Louth, LN11. 

1. Could you please go as far back in time as possible as we are investigating a latent damage to underground apparatus at this location.

2. Please ensure you provide details such as: company names, work ref, Local Authority reference, works start/end dates, works locations, description of the works and whether in footway or carriageway.
