In relation to the footway slurry sealing preparation work undertaken on behalf of Lincolnshire County Council in Stamford, Lincolnshire on 20/07/2022, please provide responses to the following:

1. Our risk assessment is referred to as a hazard awareness which we provide to the contractor.
2. Please see attached a copy of the hazard awareness document.
3. The hazard awareness form was completed by a qualified individual.

Within the Highway Works term service contract there is a specific PI that monitors the quality assessment of work. 13. We are only able to search information readily available which relates to the search criteria damage to property. The results for the past 12 months are; there were twelve reports of damage to property of any kind of surface dressing on either road or footway.
14. Further to question 13. You specifically request information related to footway preparation work. The reports are only recorded as damage to property and are not specific to footway preparation. In the event a person is reporting damage to property and is requesting to make a claim, we identify which contractor is completing the work and we refer the complainant to that contractor.
