 Please send me:

1) Proof of inspection of the road Martin South Drove B1191 in the first quarter of 2023

a) Evidence of the reported road defect with FixMyStreet reference 454864 and 454755 was not a risk to road users and if the road defect was logged why this was not actioned to be fixed [Reported via mobile in the Pothole on road/cycleway category anonymously at 20:34, Sunday 9 April 2023 53.1336230, -0.2706500] 
                                       
b) [Deep pothole reported via mobile in the Pothole on road/cycleway category anonymously at 15:41, Saturday 8 April 2023]

c) The risk assessment of the road defect was not to road users.

d) Evidence of measurements taken of the road defect at longitude and latitude 53.1336230, -0.2706500 respectively with a photograph with a object in the frame as a reference for size such a water

bottle or a tape measure.Width x Length x Depth at alleged time of inspection with date and time.

I would like the above information to be provided to me as in a reasonable time frame.

1) Attached is the site history report which will show all inspection information.

a) The site history report will show these enquiries, but they are reported by different MOPs so assume they are just looking at what is logged on FMS. 454864 and 454755 are for the same defect, a job was raised - 5704461 and fixed (which the FMS report shows). Attached is a photo of the defect (martin south drove photo 1) and a photo of it after the repair (martin south drove photo 2). 454864 does relate to a claim.
