Hubbard's Bridge manhole repair - Find a freedom of information request

My question under FOI is please can you tell me:



1) if this manhole cover has been reported as being faulty/dangerous.

2) What was done

3) and if any work was done when was it done.

I have detailed below the information that is being released to you. 

1. Yes, it has been reported – Please see attached document

2. Reported to BT Openreach as a 2hour defect under s81 of the New Roads & Streetworks Act.
