These details to include the date, location, harm/damage caused. works carried out to prevent a recurrence and any photographs and reports outlining the condition of the road at the time of each of the incidents.

• 06/01/2024 – Location – Crowtree Lane, centre of carriageway, Damage – Damage to wheel, Works Carried Out – Pothole repaired under works order 5743858 on the 23 January 2024. Please see before and after photographs attached.

• 02/07/2024 – Location – Crowtree Lane, side of carriageway, Damage – Damage to tyre, Works Carried Out – Pothole repaired under works order 5779337 on the 9 July 2024. Please see before and after photographs attached.
