1) Commissioning responsibilities are held by a single lead commissioner who oversees all adult social care placements. Alina Hackney Head of Commercial services Alina.Hackney@lincolnshire.gov.uk
2) Placements are based on assessed individual need under the Care Act 2014 with consideration given to the complexity of dementia related needs as part of a broader holistic assessment. Where appropriate, input from health colleagues and CHC eligibility may also inform placement decisions.
