A. In relation to your report by your HO, Please advise - how a verge defect of the extent that caused my puncture - could have been missed by your highways officer ?
B. The 'survey' that was done on 7/4/25 clearly missed the severity of the issue - I would like to see your evidence to the contrary?

2. What time did the inspection take place?
3. What was the job title of your Highways officer - was he a qualified highways surveyor or inspector or simply a member of highways maintenance?
4. How long has the Highways Officer(HO) - been at the LCC undertaking this 'inspectors' role?
5. What qualifications and experience did the HO have to enable the member of staff to undertaking this role?
6. What training was undertaken in order to undertake the role of inspecting the highway?
7. What speed was the vehicle driven at the time of the inspection and what type of vehicle was used?
8. What were the weather conditions at the time of passing the defect location that I reported?
9. Why did the HO decide that the location wasn't a safety issue - on what basis?
10.Please provide a copy of the risk assessment for this location as to how the decision was taken not to undertake an urgent repair.?
11. How much surface water was present in the edge defect /run-in at the time of inspection?
12.Can I have a copy of the vehicle's filming at this location?
13. Why wasn't the location identified as a 'hazard' by traffic cones, pins & tape or yellow paint? - Decision

I can confirm that the information requested is held by Lincolnshire County Council. I have detailed below the information that is being released to you.
1. A Lincolnshire County Council bespoke inspection vehicle.
2. The specific times of inspections are included within the Site History Reports. Any defects recorded also included, listed under each inspection. No further information is held.
3. Senior Highways Officer.
4-6. Qualifications and personal information are exempt under FOI. The training records of anyone carrying out highways safety inspections are kept up to date to maintain their competence. All officers carrying out inspections and being designated “Inspector” will be required to complete Highway Safety Inspectors Modular Training Assessment approved by LANTRA Awards.
7. The maximum speed for an inspection to be driven at is 40mph, however if the speed limit is lower than the inspection will be driven in accordance with the speed limit and conditions.

9. This defect was not observed as meeting the intervention threshold for repair based on Lincolnshire County Council's Highway Infrastructure Asset Management Plan.
10. Inspections are carried out on an electronic system and the record of completion of inspection of the relevant asset as well as any observations and safety defects recorded is included within the site history report. No further information is recorded.

12. This inspection was not visually recorded.
13. As per part A and request 9, this defect was not observed as meeting the intervention threshold for repair therefore was not marked up as a hazard.
Repair raised on 15th April 2025 had not reached an intervention threshold but was within the parameters to require repair prior to the next monthly inspection so a repair was ordered.
