Upload cause of death certificate

We can only accept a medical certificate of cause of death (MCCD) uploaded through our online portal. 

All MCCDs are checked for suitability to ensure they are sufficient for a registration to be completed. Follow the government guidance to ensure the MCCD is accepted and avoid delays. 

Any incorrect or incomplete certificates will be returned with the requirements for amendment indicated. Please ensure that the certifying doctor initials any modifications.

Next steps

We will notify the next of kin that they can book an appointment via text or a phone call once the paperwork has been checked.
