- to request a Mosaic user account for a worker who is new to Mosaic and your team
- to update a Mosaic role profile for a new team member
- to close a Mosaic user account when someone no longer requires access to Mosaic
- when a dual role is required. This is for workers who require an additional Mosaic user account as well as their current one
This form must be authorised by a manager.
