Request information about an incident

If you have experienced a Lincolnshire Fire and Rescue incident, or are representing a person who has (an insurance company, a solicitor or a loss adjuster for example), you can request:

  • an incident report
  • an informal discussion/ interview with the Fire Investigation Officer
  • an investigation report

Requesting an incident report

To request an incident report, please complete the application form below. If you are acting on behalf of an injured party, please also upload a “Data Subject Declaration” if applicable.

The document will contain identifying details of the location attended, the time and date of the event, the main cause (where known) and a summary of any damage. Please see sample report.

Please note, an incident report can take up to four weeks to be made available. A report for a complex incident may take longer.

Requesting an investigation report or interview

You may request a full report and/or interview with the Fire Officer when a fire has met the threshold for a more in-depth investigation. In these cases, you can request one or both of these.

The investigation report is available for specific fire incidents where the fire’s cause and origin have needed to be determined by a trained investigation officer. These investigations are more in-depth than a basic analysis. Please see sample report.

A conversation can also be requested with the Fire Investigation Officer. This can be an informal discussion of up to fifteen minutes (which is free of charge) or full interview at a cost of £330.36.

Please note, timescales for this may vary depending on the officer’s availability but we will aim to contact you as soon as possible. A report can take up to twenty-eight days to be compiled after the incident.

There is a charge of £75 for each report payment prior to release of a document. A £10 administration fee is also applicable where an invoice is requested. There is no additional cost for a card payment.
