Mental health social care needs

We support adults aged 18 and over with a wide range of needs, including physical disabilities, learning disabilities, mental health conditions, and age-related support needs. 

Our aim is to help people live as independently as possible and remain active within their communities.

We put people at the centre of their care, listening to what matters most and finding the right support together. This may include advice, assessments, care services, technology to support people at home, and help for unpaid carers.

We:

  • promote wellbeing and independence
  • help prevent needs from increasing
  • support people to stay connected to their communities
  • build on people’s strengths, networks and local support
  • help people achieve what matters most and live fulfilling lives

We work with health services, local groups, families and carers to make sure people get the right support for them, helping them in their day-to-day lives.

Most people in Lincolnshire are able to remain independent in their own homes without support from social care. If you or someone you know has tried to stay independent but needs more support, please get in touch.
