Education, health, care needs assessment (EHCNA)
Most children and young people have their needs met in a mainstream educational setting using the Graduated Approach.
Your SENCo will discuss your child's needs with you and all other supporting professionals.
You may feel that your child or young person is not making enough development with their SEND support.
If so, the school or educational setting, or you, may consider a request for an Education, Health and Care (EHC) needs assessment.
If you live in Lincolnshire, you can ask us for an EHC assessment if you are:
- the child’s parent
- a young person over the age of 16 but under the age of 25
- acting on behalf of a school or college (with the knowledge and agreement of the parent or young person where possible)
You do not need a medical diagnosis to apply.
Lincolnshire SEND uses the Lincolnshire EHC Hub, which is an innovative digital platform for families, professionals and education settings.
The platform engages with settings to contribute and collaborate on EHC needs assessments, plans and reviews.
The Hub provides a secure, shared area for everyone involved with a child or young person to make their contribution, see the contributions of others and all work towards a positive outcome. The Hub also provides the opportunity to access information at any point in the process and at any time of the day.
This approach will promote the voice of the child being at the heart of EHC needs assessments and plans, and provides a true narrative for the understanding of a child or young person’s needs and aspirations.
To submit an EHC needs assessment request , you won't need log in details, just click the "request needs assessment" on the login screen and follow the instructions to access the form.

A panel will consider the request at an allocation meeting. It will decide if an assessment is necessary and let you know within six weeks.
If it is agreed to progress to an assessment, you will be allocated a SEND caseworker.
